TOPIC: You Are an 'Exempt' International Organization

Hello Guys, I’ am new in the forum and Liberty House law club , I have a question in regarding this letter showing bellow that we have to send to Department of Public Health Bureau of Health Statistics - Bureau Chief, as I am holding a Certificated of Naturalization as US CITIZEN so do I have to address this letter to them or should I send to HOMELAND SECURITY Immigration services where I sworn in and got my certificate in New Jersey .
Also in this letter, what should I put to replace :Copy of Hospital Birth Certificate or a Bible entry” because I did not born in US and what is Bible entry ?

Hey everyone! Found an article i read pretty interesting a blogger stated to file for another ss card and in box 18 in "other" state "Authorized Representative"
What I did was made copies of my driver's license, and old ss cards and I attached "my affidavit of international organization..." and asked them to liquidate all accounts.
I then signed it like this:
Authorized Representative of the corporate JOHN DOE
____________________
John Doe
I sent this to the local SS office, I let you know what happens!
The theory... JOHN DOE is a US citizen corporation, John Doe is a citizen of Pennsylvania state. Applying for a ss card as the "authorized representative" you are declaring that your corporate JOHN DOE is in fact a U S citizen and you are in control of that corporation.

Reply: With the updated process we are using now, we don't need to send anything to the department of vital statistics. Get the latest updates and stay abreast. The only thing we really need to send out is the non-UCC to the Departments of Labor (federal and state) with an informational copy to the chief justice of the District Court of the United States for the region you are in. I have a another question to clarify about the Trustee Department
I am in New Jersey so my region is region 3 , Now as per my searches the The United States Trustee for Region 3 serves the federal judicial districts established for Delaware, New Jersey, and Pennsylvania. The regional office is located in Philadelphia, PA.
So should I send my NON-UCC to Philadelphia or to Trenton NJ ?.

Reply: Val, you have not given enough information in your question to give a proper reply. What's in NJ? Where is the US District Court for your region? All that aside, we are currently sending a certified copy of the non-UCC with a cover letter to the Chief Justice for the US District Court serving the region you are in. another 'cc' to the U.S Trustee is not mandatory but worthy of consideration. Some Districts don't list the judges for the bankruptcy court, some do.
